A (2D) image is basically a 2 dimensional array of pixels. Most of the common image operations are manipulations of this 2 dimensional data. Matlab offers a rich library of functions for vector and matrix (2d array) manipulation. Thus it is often used for quick prototyping and testing of image processing algorithms and heuristics. Fundamentally, there is nothing special about Matlab & Image processing. You can write pretty much any algorithm in Matlab, like you can do in C++ or Java.
